This series contains survey response data on controllers who took training through the Aeronautical Center. Response data was captured on two forms, identified as Form A and Form B. Form A data includes biographical data including subject identification number, study group that includes air traffic controllers located enroute and at terminals, date of birth, region, facility type, current job, promotion\ndates and grade, job transfer data, hours of sick leave used per year, symptoms, disciplinary actions, separation type codes, date of separation, and performance ratings. Form B data contains data on performance ratings.\n
